- Notice of Privacy Practices Acknowledgement (29 Kb PDF) Complete this form to acknowledge you have received information on the SHS privacy practices.
- Pharmacy Vaccine Order Form (58 Kb PDF) You will select your vaccines from this form.
- Pharmacy Vaccine History Form (41 Kb PDF) This form must be completed prior to receiving a vaccine in the Pharmacy.
Immunizations will be given:
- Monday - Friday
- 8:30am - 4:00pm
Individuals requesting an immunization must be
- 18 years of age or older
and either an
- Ohio State student
- dependent of an Ohio State student
- Ohio State faculty and staff
and cannot be obtaining an immunization in order to meet an
- Ohio State degree program requirement**
- outside requirement involving the completion of paperwork**
Available immunizations include:
- Flu
- Pneumonia
- Meningitis
- TD (Tetanus & Diptheria)
- TDap (Tetanus, Diptheria, & Acellular Pertussis)
To receive an immunization:
- No appointment is necessary.
- Go to the first floor registration desk in the Wilce Student Health Center.
- You will be asked to complete the documents to your right. (Feel free to download and complete the forms in advance.)
- Payment will be collected.
- You will be sent to the pharmacy where a pharmacist will administer the immunization(s).
- You will be asked to wait 10 minutes to ensure you do not have a reaction.
**Immunizations for requirements can be scheduled with Appointments.
