Health Information Services
Health Information Services maintains all records of patients seen in the Student Health Center. Records are retained for 7 years after the last patient visit or 8 years after a patient reaches the age of 18, whichever is later.
With proper patient authorization, the patient may obtain a copy of his/her medical and/or immunization records for a nominal fee. Requests for information are completed upon receipt within 7-10 working days.

- Monday - Thursday, 8:00AM to 6:00PM
- Friday, 8:00AM to 5:00PM
- Saturday, 9:00AM to 1:00PM
Breaks and Summer Quarter- Monday - Friday, 8:00AM to 5:00PM
